    How to claim Liberty Health Insurance?

    To claim health insurance plans from Liberty General Insurance, you can communicate through Vipul Med Corp TPA. The insurer is committed to settle your claims at the earliest and guide you when you need help the most.

    Cashless Claim Procedure

    For cashless claims i.e. health insurance claims where the insurance provider settles the claims directly with the hospital; the insured needs to take the treatment in a network hospital. To know how to register for cashless claims with Liberty Health Insurance check the steps as given below:

    Step 1: Within 24 hours of hospital admission, you need to inform Liberty General Insurance’s TPA. For Senior Citizens, you can intimate the TPA at their Senior Citizen helpline number. When lodging your claim do mention your membership number given on your health card. 

    Step 2: Go to the Insurance Company’s Hospital/TPA Helpdesk and Fill the first section of the cashless claim request form and hand over the signed Pre-Authorization form to them to fill in the remaining details.  You can find the cashless claim request form online on their TPA i.e. Vipul website or the Insurer’s site

    Step 3:  You need to fax the cashless claim request form with medical records to the Hospital to initiate the claim settlement process. 

    Step 4: The TPA will then verify all your documents and convey its decision to the hospital regarding your claim acceptance or rejection. The insurer can also ask for additional documents, in case it is required.

    Reimbursement Claim Procedure

    To file a health insurance reimbursement claim with Liberty General Insurance you need to follow the procedure as given below:

    So, if you have not availed a cashless treatment in any of the network hospitals or you have taken treatment in a hospital that does not come under the Insurer’s network, then all the medical treatment bills and documents need to be submitted to the TPA.

    You need to settle the bills with the hospital and then submitted them to the Third-party Administrator and claim for reimbursement.

    Rest all the procedure remains the same as cashless treatment, except the fact that you need to settle the bills at the hospital and then file for its reimbursement.

    What are the Documents Required for Liberty Health Insurance Claims?

    Given below is an indicative list of documents that are required as per your claim requests: 

    In-patient Treatment/Daycare Procedures

    • Duly filled and signed Liberty health insurance claim form
    • Photocopy of current health plan/ID card
    • Collect the daycare/discharge summary from the hospital
    • Hospital bills with bill number to be signed by the insured
    • Submit the Original hospital payment receipt with the receipt number
    • Also, keep all the consultation letters and prescriptions
    • Submit all the original bills, payment receipts, and doctor’s reports suggesting such test
    • Certificate from the surgeon stating the nature of Operation/Surgeons Bills/Receipts
    • Certificate from the attending doctor/ specialist/ consultants/ anesthetist bill and receipt
    • Original invoice/bills for any Implants with original payment receipts
    • Doctors registration Number, Hospital Registration Number and PAN details from the Hospital

    Road Traffic Accident Claims

    • Copy of the Policy FIR Report from / Copy of the Medico-Legal Certificate

    In Accidental Death cases

    • A copy of the Death Certificate/ Post Mortem Report (if conducted)

    For Death Claims
    Apart from in-patient treatment documents:

    • You also need to collect and submit the original death summary from the hospital
    • A copy of the death certificate
    • A copy of the Legal heir certificate

    Pre and Post-hospitalization expense claims

    • Duly filled and signed Liberty health claim Form
    • Xerox of current Liberty health insurance policy or ID card
    • Original receipts/medicine bills/consultation bills with prescriptions
    • A copy of the discharge summary of the main Liberty health claim

    Ambulance claims

    • Duly filled and signed liberty Claim Form
    • Xerox of current Liberty health insurance policy or ID card
    • Original bills with payment receipts
    • Treating doctor’s consultation suggesting emergency hospitalization 

    Reimbursement of Organ Donor Expenses
    In addition to the above documents you need to furnish the following documents:

    • A blood test or organ function test proving the organ failure
    • Treatment Certificate from the Hospital’s Transplant Surgeon
    • Original bills and payment receipts of Nursing charges

    The insurer may call for additional documents/ information as relevant to the claim.
