The benefits under this policy are provided via four different plans, which are Value Healthline, Freedom Healthline, Enrich Healthline and Privilege Healthline.

      • The sum insured amount ranges from Rs 2 lakh to Rs 10 lakh
      • Pre and post hospitalization expenses covered for 30 and 60 days respectively
      • Daycare procedures are covered that do not require 24 hours hospitalization
      • Domiciliary treatment is covered where the treatment is undertaken at home for a maximum of 7 days in a single policy year
      • Emergency ambulance charges up to Rs. 1000 are covered per hospitalization
      • Treatment expenses for the organ donor at the time of organ transplant are covered

        • Benefits received under this plan is for a floater Sum Insured value up to Rs. 15 lakh
        • 141 Daycare procedures are covered that do not require 24 hours hospitalization

        • Emergency ambulance charges are covered up to Rs. 1000 to Rs. 3000, depending on the type of plan chosen

      • Chola MS Critical Healthline Insurance Plan

        This is a fixed benefit health insurance policy with flexible sum insured options for listed critical illnesses that are mentioned in the policy. The features of the plan are as follows:
        • Lump sum payment for a range of critical illnesses
        • Sum insured amount ranges from Rs. 3 lakhs to Rs. 50 lakhs
        • There are two sub-categories of plans to choose from which are the Standard and the Advanced Plan. The Standard Plan covers ten critical illnesses, while the Advanced Plan covers 12 critical illnesses plus ambulance charges up to Rs. 1000.
        • The critical illnesses covered under the Standard Plan are as follows – cancer of a specified severity, stroke resulting in permanent symptoms, first heart attack of specified severity, open chest CABG, kidney failure requiring regular dialysis, Multiple Sclerosis with persisting symptoms, major organ/bone marrow transplant, permanent paralysis of limbs, surgery of aorta, primary pulmonary arterial hypertension.
        • The Advanced Plan covers all the above critical illnesses plus Parkinson’s Disease and Motor Neuron Disease with Permanent Symptoms

      • Chola Hospital Daily Cash Allowance

        This is a plan that provides for a specified amount every day for which the insured is hospitalized to take care of the family’s financial requirements. The policy is further sub-categorized under six different plans. The features of the plan are as follows:
        • Hospital cash benefit for normal hospitalization is provided in which a daily cash amount is paid for each continuous and completed period of 24 hours if the insured person is hospitalized for more than 24 hours, due to sickness.
        • Hospital Cash for ICU hospitalization is provided in which twice the daily amount is paid for each continuous and completed period of 24 hours if the insured person is hospitalized for more than 24 hours due to sickness.
        • A convalescence benefit is provided in the form of a lump sum amount if the insured person is hospitalized for more than 20 continuous days

    Both cashless and reimbursement claims can be filed for Cholamandalam health insurance plans. The process is explained below:

    Cashless Claims

    Here the Insurer directly pays network hospital and the process is given below- 

    • Call Chola Help desk at 1800-208-5544 within 48 hours in Emergency & 72 hours prior to Planned Admission
    • The insurer may ask for:
      • Policy number
      • Date and time of hospitalization
      • Name of the insured
      • Location and nature of the problem
      • and your contact details to register a claim

    Admission in a network hospital

      • Approach Hospital’s Insurance desk for Pre-Authorization approval
      • Show your insurance policy and provide valid photo id proof
    • Hospital will send the cashless request to Chola and coordinate for authorization

    Authorization and discharge

    • At the time of Discharge, Hospital will share all the necessary documents with Chola for approval
    • Chola will review the shared documents and give the authorization on the final bill amount as per policy terms and conditions after completion of all formalities

    Reimbursement: Pay hospital and then claim for medical expenses from insurer.

    Inform Chola MS

    • For planned hospitalization, call the Help desk on 1800-208-5544 within the stipulated time

    Hospital Admission

      • Get the insured admitted and pay directly to the hospital
    • On getting discharged, please collect all the documents, medical reports, invoices, and discharge certificate in original from the Hospital

    Reimbursement claims

    Within 30 days after discharge file for the reimbursement claim with your insurer with:

    • Duly completed and signed Claim form, in original
    • Original Discharge summary in the hospital letter head with the seal and sign of the doctor with treatment details
    • Original Main bill from the hospital with cost wise break up
    • Original payment receipt and investigation reports
    • All pharmacy bills should be accompanied with relevant prescriptions
    • Implant stickers or invoice where ever applicable
    • In case of Road traffic accident (RTA), copy of FIR and/or Medico legal Certificate (MLC) would be required.
    • Identity proof and residence of the beneficiary for claims above Rs 1 Lakh
    • After the claim is approved, the insurer will pay the reimbursement as per policy terms and conditions. Insurer may call for additional documents, if required.
