      Steps to File a Reliance Health Insurance Claim

      As soon as the doctor advises hospitalization, the policyholder must intimate Reliance General Insurance Company about it. The policyholder can either raise a cashless claim by visiting the network hospitals of the insurance company or a reimbursement claim by visiting a non-network hospital. Follow the steps given below to file a claim under a Reliance Health Insurance policy:

      Claim Intimation:

      Information to be kept ready while intimating the claim to the insurance company are as follows:

      • Policy number
      • Insured/claimant’scontact details (phone no., email id, address, )
      • Name of insured/claimant, who is hospitalized
      • Relationship of the policyholder with the person who is hospitalized
      • Name of the hospital
      • Nature of the ailment
      • Nature of the accident (For accident cases)

      • Commencement date of the symptoms of the ailment

      Claim Process for Cashless Facility:

      Steps to file a cashless claim under Reliance Health Insurance Plans are as follows:

      • Get admitted to a reliance General Insurance Company’s network hospital
      • Show the Reliance Health Card at the hospital
      • Fill up the "Cashless Request Form" available at the Third Party Administrator (TPA) desk at the hospital
      • Submit the filled-in pre-authorization form along with a copy of the Reliance health card to the TPA while getting to the hospital (Remember to carry a photo ID card)
      • The hospital sends the pre-authorization form to RCare Health and waits for their authorization.
      • Get treated at the hospital after the authorization is received
      • The insured/claimant should verify and sign all the bills during discharge.
      • Leave the original hospital discharge summary, investigation reports and other documents with the hospital after keeping a copy of these documents for record
