Ways to File a Liberty Health Insurance Claim

The policyholders can file claims with Liberty General Insurance in two ways i.e. cashless and reimbursement claims.

      1. Cashless Medical Insurance Claims

Liberty General Insurance facilitates an efficient cashless claim process at its associated network hospitals. Policyholders can obtain necessary medical treatments, and the medical insurance company will pay the bill directly to the network hospitals, which is especially beneficial in urgent care situations. The process is designed to be both quick and user-friendly. At the hospital, insured individuals are required to provide their medical insurance policy details to avail of the cashless facility.

      2. Reimbursement Medical Insurance Claims

In cases where treatment is obtained from hospitals outside Liberty General Insurance Company's network, they offer a straightforward reimbursement process. Patients settle their medical expenses first and then seek compensation from the insurer. This approach provides the flexibility of choosing from various healthcare providers and a streamlined process for claim filing and settlement. It is important for policyholders to systematically gather and submit all relevant medical documents and bills to process their reimbursement claims effectively.

How to File a Claim with Liberty Health Insurance?

Liberty General Insurance offers two methods for filing claims for their health insurance plans. Here's how you can proceed with cashless and reimbursement:

      1. Liberty Health Insurance Cashless Claim Process:

Follow the steps below to navigate the cashless claim process with Liberty Health Insurance:

        Inform the Insurer

As soon as hospitalization is required, contact Liberty General Insurance within the specified timeframe: 48 hours for emergency hospitalization and 3 days before planned hospitalization. This can be done by calling their toll-free number, sending an email, or mailing a claim intimation letter to their appointed TPA or relevant branch office.

Presentation of Identification and Policy Details

Present your Liberty Health Card or policy number along with a valid ID proof like a passport or PAN card at the hospital's TPA desk. This step is essential for identity verification and to facilitate the pre-authorization process.

        Submission of Pre-Authorization Form

Fill out the pre-authorization form available at the hospital and submit it along with the doctor’s consultation papers within the stipulated timeframes. The hospital’s TPA desk will forward this to Liberty General Insurance for processing.

Processing of Cashless Claim by Insurer

After receiving your documents, Liberty General Insurance will assess and classify the case as authorized, queried, or denied. They will communicate the decision to you and the hospital, usually via email and SMS.

        Direct Claim Settlement to Hospital

Once your treatment concludes, the insurance company directly settles the bills with the hospital (as per policy terms & conditions).

      2. Liberty Health Insurance Reimbursement Claim Process:

Outlined below is the procedure for filing a reimbursement claim with Liberty Health Insurance:

        Prompt Notification to the Insurer

If you get admitted to a non-network hospital, inform Liberty General Insurance within the specified timeframe: 48 hours for emergency hospitalization and 3 days before planned hospitalization.

Payment of Hospital Bills and Collection of Documents

After receiving treatment, you need to pay all hospital bills yourself and collect all relevant documents, including medical reports, invoices, and discharge summaries.

Submission of Claim Documents to the Insurer

Complete the claim form and submit it, along with all pertinent documents, to Liberty General Insurance within 10 days of discharge.

        Claim Assessment

The insurer will review your submitted claim and decide to approve, query, or reject it based on the policy terms and conditions. This stage involves thoroughly examining all provided documents to ensure compliance with policy norms.

        Claim Processing

The claim can be finalized as per the policy's terms and conditions, and the sanctioned amount will be reimbursed via NEFT.
