    Family floater means the sum insured shall be floating over the insured members. Here the family means self, spouse and 2 dependent children.

    Key Features of Future Health Suraksha Family Floater Insurance Plan:

    • The family floater is divided into 5 categories
      • 2 Adults
      • 2 Adults + 1 Child
      • 2 Adults + 2 Children
      • 1 Adult + 1 Child
      • 1 Adult+ 2 Children
    • Each category consists of a Basic Plan, Silver Plan, Gold Plan and Platinum Plan.
    • Pre-acceptance medical tests are not required for all proposers up to the age of 45 years irrespective of the sums insured.
    • All pre-acceptance medical tests will have to be done in Future Generali empanelled diagnostic centers only.
    • In-house cashless settlement is provided.

    Future Generali Health Suraksha Family Floater Insurance Policy Benefits:

    • With Health Suraksha, the member has access to cashless facility at the empanelled hospitals across India.
    • Pre and post hospitalization expenses incurred from 60 days prior up to 90 days after hospitalization.
    • Ambulance charges up to Rs.1500.
    • 130 Day-care procedures.
    • Hospital cash benefit for Platinum Plan.
    • Pre-existing disease covered after four years of consecutive renewal.
    • Accompanying person expenses up to Rs.500 per day.
    • Additional accidental hospitalization limit enhanced by 25% of balance sum insured, subject to a maximum of Rs.1 lakh.
    • 50000/- Sum Insured would be available for dependent children only.
    • Porting of similar health policy to Future Health Suraksha as per portability guidelines.

    Policy Details

    Sum Insured: Up to 10 Lakhs

    • In case of entry above 55 years- the maximum sum insured is 5 lakhs.

    No Claim Bonus: Cumulative bonus of 10% on the basic sum insured will be provided up to a maximum of 50% of the sum insured.

    Coverage type: Family Floater

    Grace period: Grace Period of 30 days for renewing the policy is provided under this policy.

    Renewal Policies: The policy if renewed continuously without any break will be renewed lifelong.

    Policy termination: You may cancel this insurance by giving at least 15 days written notice, and if no claim has been made then premium will be refunded on short term rates for the unexpired Policy Period as per the rates detailed below.

    Additional features or riders

    • Discounts:
    • Family discount of 10% is available in case more than one person is covered in the same policy.
    • Premium paid by any mode other than cash is eligible for tax relief as provided under Section 80-D of the Income Tax Act.

    Exclusions

    • Any ailment, surgery or condition which has been diagnosed 48 months before the inception of the policy coverage.
    • Any ailment or disease which is contracted within 30 days of commencement of the policy.
    • Non-allopathic treatment
    • Congenital diseases
    • All expenses related to AIDS and related diseases.
    • Joint replacement surgery is covered after a waiting period of three years, unless it happens due to an accident.
    • Use of intoxicating drugs or alcohol

    Documents Required For Future Generali Health Suraksha Family Floater Insurance Plan:

    • Original Bills (including but not limited to pharmacy purchase bill, consultation bill, and diagnostic bill) and any attachments thereto like receipts or prescriptions in support of any amount claimed.
    • Reports include all medical reports, case histories, investigation reports, treatment papers, discharge summaries.
    • A detailed list of the individual medical services and treatments provided and a unit price for each.
    • Prescriptions that name the Insured Person and in the case of drugs: the drugs prescribed, their price and a receipt for payment. Prescriptions must be submitted with the corresponding Doctor’s invoice.
    • Previous treatment record along with reports.
    • MLC/ FIR copy/ certificate regarding abuse of alcohol/intoxicating agent, in case of Accidental injury.

    Eligibility Criteria For Future Generali Health Suraksha Family Floater Insurance Plan:

    • Minimum Age- 90 days
    • Maximum Age- 70 years
    • Children above age of 90 days are eligible if either parent is getting insured under this policy.
    • Children will be covered as dependents’ up to 25 years of age.
