      Why Was Janani Suraksha Yojana Needed?

      In 2005, a large number of deliveries in India, especially in rural and poorer households, were still taking place at home without any skilled medical support. No doctor. No trained midwife. No emergency care if something went wrong. The consequences were devastating. Mothers died of complications that a timely hospital delivery could have prevented. Newborns died of infections and birth-related emergencies that basic medical attention could have managed.

      The problem was not just awareness. It was money. Getting to a hospital costs money. Staying there costs money. Taking time off work costs money. For a BPL family in rural Bihar or Chhattisgarh, these were not small considerations; instead, they were decisive barriers. JSY was designed to break down those barriers, one cash transfer at a time.

      What Is the Objective of JSY?

      JSY is not a single-purpose organisation. It was constructed on a series of outcomes:

      • Incentivise pregnant women to go to government hospitals or private hospitals that have been approved.
      • Decrease maternal mortality and neonatal mortality in India.
      • Support families with financial assistance when they are unable to assist with delivery costs.
      • Increase awareness about antenatal care (ANC) and safe delivery practices.
      • Engage pregnant women in the health care system via ASHA workers and community health networks.

      The scheme combines cash transfers with antenatal and in-labour support to mothers, institutional-supported delivery, and postnatal s. It views the woman's journey through her pregnancy, birth, and post-birth as a continuum rather than a single event.

      Who Is Eligible for JSY?

      JSY eligibility is based on age, economic status, number of births and state of residence. This is fairly transparent:

      • The mother must be 19 years of age or older at the time of delivery; she is ineligible if she is under 19.
      • She should be from a BPL (Below Poverty Line) or an SC/ST family household.
      • Benefits for only 2 live births.
      • She has to give birth in a government health unit or an accredited private health unit.
      • She should register herself as an ASHA/ANM/Health Worker at the Sub Centre/PHC/Anganwadi Centre.

      The scheme is more inclusive in Low Performing States (LPS), wherein all women delivering in government health facilities are included.

      LPS vs HPS: What Is the Difference?

      JSY divides all states into two categories according to the percentage of institutional deliveries:

        • Low-Performing States (LPS) - These are the states that have historically performed poorly in institutional deliveries: Uttar Pradesh, Uttarakhand, Bihar, Jharkhand, Madhya Pradesh, Chhattisgarh, Assam, Rajasthan, Odisha, and Jammu and Kashmir. These states are focused on and receive more cash assistance.
        • High Performing States (HPS)- All other states and Union territories are considered to be in the High Performing States (HPS) category. Lower cash assistance amounts may be due to the relatively better institutional delivery rates.

      Many people say this classification is smart because it points out the most support where it's needed most.

      How Much Cash Assistance Does JSY Provide?

      The cash amount varies based on where the woman lives and which category her state falls under:

      In Low Performing States (LPS):

      • Rural areas, ₹1,400 to the mother
      • Urban areas, ₹1,000 to the mother

      In High Performing States (HPS):

      • Rural areas, ₹700 to the mother
      • Urban areas, ₹600 to the mother

      ASHA workers also receive an incentive for facilitating the delivery:

      • In rural LPS, ₹600 per delivery
      • In urban LPS, ₹400 per delivery
      • In HPS, ₹200 per delivery

      The cash is paid to the beneficiary directly at the time of delivery in the respective health institution, not after and also not through a third party. The money goes to the mother.

      The Role of ASHA Workers in JSY

      In fact, JSY would not exist without ASHA. This is not an understatement. The ASHA is the individual who operationalises this scheme at the grassroots level, particularly in remote and inaccessible areas.

      Under JSY, ASHA workers in the L.P.S. and North Eastern states have a set of responsibilities:

      • Recognising pregnant women in their area and assisting them to register.
      • Taking the pregnant woman to the hospital for the birth of the child.
      • Remaining with the mother, during and after the birth.
      • Supporting the post-natal management, checking the mother and infant within 48 hours after delivery.
      • Providing breastfeeding, hygiene, immunisation and post-delivery warning education to the families.

      There should be a JSY card and an MCH (Mother and Child Health) card for each pregnancy registered under JSY. The micro-birth plan, prepared by an ASHA worker for each beneficiary, is an easy-to-use tracking sheet for the beneficiary's antenatal visits, planned delivery facility, and postnatal follow-up.

      What Documents Are Needed to Apply for JSY?

      The documents required are basic:

      • Aadhaar card or Voter ID (identity proof)
      • BPL card, poverty certificate, or SC/ST certificate
      • Pregnancy registration slip from a government health centre
      • Bank account details for direct benefit transfer
      • MCH-JSY card issued at registration

      How to Apply for JSY

      It's simple and can be performed offline or online:

      • Offline: Go to the nearest government health centre (GHC), Sub Centre, PHC or Anganwadi. Get in touch with your area ASHA worker or ANM. Provide documents as needed. Registration is done at the health institution. Cash paid upon delivery.
      • Online: JSY online registration is offered through the state NHM online portals in many States. Go to the state's JSY website, enter personal and pregnancy information, upload documents, and complete the process. Please ensure all bank information is correct for direct transfer.

      JSY and JSSK, A Powerful Combination

      JSY is highly successful when combined with Janani Shishu Suraksha Karyakram (JSSK), which provides free drugs, diagnostics, blood tests, transport to and from the hospital, and treatment for sick newborns in government health facilities.

      As part of the JSY cash, a BPL woman delivering at a government hospital can practically avail an institutional delivery at a zero out-of-pocket cost. Transport is included in Janani Shishu Suraksha Karyakram. Medicines are also free. The delivery is included in the price. And JSY cash is available to support incidental expenses. Both are part of a full-fledged safety net for poor mothers and their newborn children.

      Impact of JSY So Far

      The numbers speak for themselves:

      • Over 10 crore women have accessed institutional delivery under JSY since 2005
      • Institutional delivery rates in low-performing states have risen significantly

      A mother who delivers in a hospital has access to a skilled birth attendant, emergency obstetric care, immediate newborn support, and post-natal monitoring. These are not luxuries. They are the minimum conditions for a safe birth. JSY, by making institutional delivery financially viable for the poorest families, has moved millions of mothers and newborns from a situation of risk to one that is relatively safe.
