The Sarbat Sehat Bima Yojana (SSBY) is the flagship health insurance scheme of Punjab, which was launched on 20th August, 2019, providing cashless health insurance coverage of Rs. 5 Lakhs per family per year at empanelled hospitals. It was enhanced to Mukh Mantri Sehat Yojana (MMSY) in January 2026 and increased its coverage to 10 lakh health cards, which Punjab families can avail annually. It has been rolled out across all bona fide households of Punjab, starting from 22nd January 2026, so that Punjab becomes the first state in the country to provide a medical cover of ₹10 lakh to all households. The scheme is implemented by the SHA Punjab (sha.punjab.gov.in), and the insurance partner is United India Insurance Company.

      What is the Need for this Scheme?

      Although Punjab has a more developed economy than many others, the risk of health emergencies cannot be avoided for anyone. In case of serious illness, private hospital costs are often a heavy burden on the daily wage earners. In Punjab, prior to 2019, most of the low-income segment was without any formal health coverage.

      While most of the working population of Punjab was kept out of Ayushman Bharat PM-JAY (limited to the families covered in SECC 2011), SSBY extended cover to Smart Ration card holders, J-form farmers' families, small traders, journalists and construction workers.

      As reported in the Punjab Budget 2024-25, e-cards were given to 35.59 lakh of the total 44.10 lakh families eligible, and approximately 2,227 crore of claims were made by the people. By 2026, MMSY was also upgraded by removing the category of coverage and, in a way, covering everyone.

      Eligibility

      Since January 2026, under MMSY, it has covered all bona fide residents of Punjab. The old SSBY scheme covered the following groups:

      • Families listed in the SECC 2011 database falling under the central scheme AB PM-JAY.
      • Holders of Smart Ration Card
      • Farmers holding J-form and registered with the Agriculture Department.
      • Small traders registered with the Excise and Taxation Department.
      • Accredited Yellow Card Journalist
      • Registered construction workers of the Punjab Building and Other Construction Workers Welfare Board

      Under the current MMSY (January 2026 onwards): Under the new MMSY (from Jan 2026 onwards), it covers all bona fide residents of Punjab (no upper income limit, no specific category, age not specified). Almost 65 lakh families comprising nearly 3 crore persons have been covered now, and around 20 lakh excluded families are covered.

      Punjab Govt 10 Lakh Health Card Apply Online – Registration Process

      You can now complete your registration for the Punjab government's 10 lakh health card online or through offline methods.

      • Through Health Camps: The Punjab government has established 128 Registration camps in every district. Take your Voter ID and Aadhaar card to the nearest camp. The registration will be finalised in 10-15 days, after which the MMSY Health Card will be issued. If someone does not attend a camp, he/she may avail the treatment by presenting Aadhaar or Voter ID at any empanelled hospital.
      • Punjab Health Card Apply Online: Visit sha.punjab.gov.in and then choose the Citizen Corner column, enter Aadhaar number and take the help of Sha Punjab eligibility check online. Fill 10 lakh health card application online to get your e-card for Ayushman Bharat through Ayushman App or beneficiary.nha.gov.in.
      • At a Common Service Centre (CSC): Navigate to sha.punjab.gov.in, and then "Citizen Corner" and "Location for E-Card Generation (CSC)". Select your district and find the nearest CSC, bring your Aadhaar and Voter ID.
      • At an Empanelled Hospital: Visit to Arogya Mitra help desk (no appointment slip required from February 2026). All staff verify and facilitate eligibility where necessary and in emergency situations. 

      Benefits and Coverage

      • Cashless Cover- Fully Cashless, No Out-of-Pocket payment— ₹10 lakh per family per year at empanelled hospitals on a floater basis.
      • Treatment Packages- More than 2000 packages, including cardiac surgery, cancer treatment, neurosurgery, kidney dialysis/transplant, orthopaedics, burns, neonatal care and emergency treatment.
      • Pre-existing Diseases - All pre-existing conditions are fully covered from the outset, with no waiting period to prove that you have no pre-existing conditions.
      • Pre & Post-Hospitalisation- Medicines/ diagnostics up to 15 days after discharge are covered in addition to a cover for Pre & Post-Hospitalisation- Expenses up to 3 days before admission.
      • Package Pricing- Rates are fixed, standardised, and do not contain any hidden costs that may arise and are the charges levied by the specialist, surgeon, and ICU.
      • Portability (SECC Families)- AB-PMJAY empanelled hospitals across India can treat the ₹5 lakh central component for PM-JAY eligible families.
      • Funding- for SECC families: 60:40 Centre/State ratio. The rest of the families in Punjab are completely funded by the state government.

      Other concurrent schemes available to MMSY beneficiaries are: Janani Suraksha Yojana (JSY) for incentives for institutional delivery, Pradhan Mantri Matru Vandana Yojana (PMMVY) for maternity cash benefit and Surakshit Matritva Aashwasan Yojana (SUMAN) for free specialist antenatal checkups. The Nikshay Poshan Yojana provides Rs 1000/- per month to the TB patients. In Government superspecialty hospitals, the family members are eligible for additional one time support of 15 lakh, if the family expenditure is beyond the limit of the scheme.

      What is NOT covered:

      • OPD consultations not leading to hospitalisation
      • Cosmetic and elective procedures not on the approved list
      • Treatment at non-empanelled private hospitals
      • Drug de-addiction and fertility treatments

      • Aadhaar card - Mandatory for identity and biometric verification
      • Voter ID card - Required at registration camps
      • Smart Ration Card - For earlier SSBY category verification if needed
      • J-form - For farmer families under the earlier SSBY framework
      • Old SSBY/MMSBY health card - If available, advisable to retain during transition

      Punjab Govt 10 Lakh Health Card Download Process

      • Ayushman App: You need to download the Ayushman App from Google Play Store. After logging in using your Aadhaar-linked mobile number, download your Ayushman Card. This is the simplest method for getting 10 lakh card downloads on your cell phone.
      • Beneficiary.nha.gov.in: To download the e-card PDF, fill in details like Aadhaar or mobile number on beneficiary.nha.gov.in, verify using OTP, and proceed with the download. Download the PDF Punjab health card here.
      • sha.punjab.gov.in: Go to "Citizen Corner" → e-card download section for your Punjab govt 10 lakh health card download pdf.
      • Disabled e-card status: Go to sha.punjab.gov.in → Citizen Corner → Check Disabled E-Cards Status and fill E-Card ID and captcha.
      • Existing SSBY cardholders: Old cards are good for the transition period until replaced with new MMSY cards.

      Claim Process

      MMSY operates in a cashless, no advance payment and no paperwork for reimbursement system.

      1. Go to any empanelled hospital with an MMSY Health Card/Aadhaar card.
      2. No appointment slip required for approaching the Arogya Mitra help desk
      3. Pre-authorisation of planned procedures is done online by the hospital before the start of treatment.
      4. Treatment is immediately commenced, and pre-authorisation is given as needed in emergencies.
      5. All the bills are paid directly from SHA Punjab to the hospital, and patients pay nothing.
      6. Post-discharge medicines and diagnostics are included in the same package for a period of 15 days.

      For families with members with Autism, Cerebral Palsy or Multiple Disabilities, they can also consider the Niramaya Health Insurance Scheme for specific disability coverage. MMSY benefits are also available for children under Mission Vatsalya and adolescent girls under the Scheme for Adolescent Girls (SAG).

      Why MMSY Matters for the General Public

      The initial scheme started in 2019, only benefiting a few vulnerable sections. Today, a universal 10 lakh bima yojana Punjab that covers every family in the state has been introduced. Whether a construction labourer from Jalandhar, or a cancer- afflicted patient, or a farmer from Ferozepur, who needs to make rounds of the kidney-dialysis centre for his ageing father, no family has to go into debt over medical bills due to the MMSY card.

      Punjab is the first state to bring a universal 10 lakh medical cover to all its citizens. However, there is scope for improvement. Most of the big private health networks, such as Fortis, are yet to be empanelled under the scheme, OPD services exclusions under the scheme are disappointing, and the district-wise launch can leave remote families to wait.
