      Why Was Rashtriya Arogya Nidhi Needed?

      Suppose you are informed that a family member needs a kidney transplant or a liver operation. The doctor's recommendation is evident. The treatment is available. However, the bill will be in lakhs compared to your family income, which is just enough to make ends meet.

      This is what happens to millions of BPL families in India. A severe condition is not an ordinary threat to health. It puts at stake all your savings, livelihoods and the future of your children. The vast majority of these families lack health insurance, have no backup plan, and nowhere to run when a disastrous medical bill comes.

      And that is precisely the loophole that Rashtriya Arogya Nidhi was developed to fill. In India, no family should be made to sacrifice the life of a loved one in the name of survival. It is not merely a moral stand and is the very principle of this plan. 

      What Is the Structure of the Rashtriya Arogya Nidhi Scheme?

      Rashtriya Arogya Nidhi is not a single fund, but it is an umbrella scheme that has three distinct components, each of which has its specific category of patients:

      1. Rashtriya Arogya Nidhi (RAN)- Core Component: Assistance in the form of money towards life-threatening diseases related to the heart, kidney, liver and others in government super-speciality hospitals. The highest financial aid amount in this component is up to 15 lakh per patient.
      2. Health Minister Cancer Patient Fund (HMCPF): This was established in 2009 and specifically targets cancer patients belonging to the BPL category. The treatment should be provided at one of the 27 Regional Cancer Centres (RCCs) across India. Each RCC has Revolving Funds of up to ₹50 lakh at its disposal. The maximum financial assistance under HMCPF is up to ₹15 lakh per patient.
      3. Scheme on Financial Assistance in Rare Diseases: This aspect of the scheme is related to the treatment of patients with specified rare diseases in government hospitals with super-speciality facilities. The limit of three elements is up to 20 lakh per patient, the highest among all three components, since the treatment of rare diseases is exceptionally costly.

      Who Is Eligible for Rashtriya Arogya Nidhi?

      The eligibility is unique and should fulfil all of them:

      • The patient should be from a family that is registered as Below the Poverty Line (BPL) according to the respective State or UT poverty line.
      • The annual family income should not exceed ₹1,25,000.
      • The patient should be receiving treatment at a government hospital or an approved medical institute under the scheme.
      • The disease should be severe or life-threatening, like heart disease, kidney failure, liver disease, cancer or a given rare disease.
      • The same treatment should not be receiving financial aid from another government or a private source.
      • For HMCPF, to treatment must be at one of the 27 approved Regional Cancer Centres.

      Who is NOT eligible:

      • The HMCPF component does not cover employees of the central government, state governments, or PSUs.
      • Patients who are undergoing cancer treatment at the hospital without any cost cannot access the funds of HMCPF.
      • Patients receiving treatment in a private hospital do not qualify in any aspect of RAN.

      How Does the Money Reach the Patient?

      It is a significant fact which numerous individuals overlook. Financial assistance from RAN is not given directly to the patient or the patient's family. The grant is sent to the Medical Superintendent of the particular government hospital where the patient is being treated.

      A sum of ₹50 lakh (₹90 lakh in the case of Medical Superintendents/Directors of AIIMS, New Delhi) has been allocated to the Medical Superintendents/Directors of 13 Central Government Hospitals and Institutes in the country as Revolving Funds. This fund enables each hospital to approve financial assistance of up to ₹ 2 lakh per eligible case, directly from the fund. Once the treatment cost exceeds ₹2 lakh, the case is referred to the Ministry of Health and Family Welfare for consideration and supplemented with the remaining funds. 

      This model will ensure that the funds are used for medical treatment and not otherwise. 

      What Treatments Are Covered Under RAN?

      The scheme includes a broad scope of medical conditions and medical procedures:

      • Cardiovascular diseases - Coronary angiography, angioplasty without a stent, bypass surgery, valve replacement surgery, and aortic surgery.
      • Kidney diseases- Renal transplant and dialysis procedures.
      • Liver diseases- Liver transplantation, Portal hypertension surgery, Hepatic encephalopathy.
      • Treatment of cancer - Anti-cancer chemotherapy, hormonal therapy, radiation therapy, Gamma Knife surgery, IMRT, brachytherapy, bone marrow transplants, and PET scans.
      • Rare diseases- Specified rare diseases that are treated in government super-speciality hospitals.
      • Other diseases include retinal detachment, glaucoma, and other severe conditions, which are listed under the RAN.

      How to Apply for Rashtriya Arogya Nidhi

      The government hospital where the patient is receiving treatment handles the entire application process. The following is the step-wise procedure:

      • Access the official RAN application form on the MoHFW website at mohfw.gov.in.
      • Complete the form properly, ensuring every part is filled in.
      • Sign and stamp the form by the treating doctor or the Head of Department (HOD).
      • The Medical Superintendent of the government hospital should also countersign the form.
      • Send the duly filled form and all supporting documents to the Medical Superintendent.
      • For cases where the treatment cost exceeds ₹2 lakh, the application is forwarded to the Ministry of Health and Family Welfare for further processing and sanction.

      For HMCPF (cancer cases), processing is done directly at the regional cancer centre of interest.

      Documents Required For Rashtriya Arogya Nidhi

      The documents are basic but must be in order:

      • A duly filled application form signed by the treating doctor and countersigned by the Medical Superintendent.
      • Copy of the ration card, attested by a Gazetted Officer
      • Income certificate confirming BPL status
      • Patient's identity proof and medical reports as required

      What Is NOT Covered Under RAN?

      Being clear about exclusions is just as important as knowing what is covered:

      • Treatment at private hospitals, RAN does not cover private hospital treatment under any circumstances.
      • Common illnesses that do not require expensive or specialised treatment.
      • Retrospective reimbursement of already-incurred medical expenses is generally not permitted.
      • If the treatment is available free of cost at the hospital, HMCPF funds cannot be used.
      • Government and PSU employees are excluded from the HMCPF component.

      How Much Has Been Disbursed So Far?

      The scheme has been making a difference at the ground level. According to PIB, ₹27.06 crore was disbursed under the Umbrella Scheme of RAN, including the HMCPF component, in 2024-25 alone. The funds are released on a case-by-case basis, both online and offline, ensuring that each patient's situation is individually assessed.

      States and Union Territories are also required to set up their own State Illness Funds, to which the Central Government contributes. States and UTs are required to contribute 50% of the total amount to their respective funds, making this a genuinely collaborative effort between the Centre and states.

      Why Rashtriya Arogya Nidhi Matters

      Behind every application filed under RAN is a family that has been pushed to its absolute limit or a father who has spent every rupee of his savings. A mother who has sold her jewellery. A family that has borrowed from everyone they know, and still cannot cover the hospital bill.

      RAN, for all its procedural requirements and limitations, the hospital-only model, the BPL restriction, and the need for multiple countersignatures remain a lifeline. And for a family that has run out of options, a lifeline is everything.
