Claim Process of United India Health Insurance

  • To avail of cashless treatment facility, you can immediately inform the insurer about your hospitalization. For cases related to emergency hospitalization, you need to notify the insurer within 24-hours of getting admitted in the hospital
  • You can collect the claim form from the TPA or download it online
  • Fill the claim form and submit the signed claim form to the TPA along with all the documents within 15 days
  • In case of cashless health insurance claims, you should submit the pre-authorization cashless form to the hospital’s TPA department
  • If the submission of the pre-authorization form is done soon after the admission you don’t need to give intimation separately
  • To get reimbursement for your expenses, you should submit all the documents and forms within of discharge from the hospital

Claim Process for Cashless Treatment

Cashless claims are only valid for policies that are serviced by a Third-party-administrator:

  • Check if the hospital where you are planning to avail of the treatment comes under the list of network hospitals. Cashless medical treatment can only be availed in the registered network hospitals
  • If it was a planned treatment, you can notify the TPA department in advance with all the details like the hospital name and address
  • If it is a medical emergency case you can immediately notify the TPA after your hospital admission
  • After taking admission to a hospital, you need to fill, sign and submit the pre-authorization form and send it to the hospital’s TPA
  • This form includes all the details related to your treatment, hospital admission, disease, and its cost estimation
  • Submit all the original documents to the TPA department and once all of it is done your claim amount will be settled at the hospital
  • If your claim request is not approved you can file for reimbursement

Claim Process for Reimbursement Treatment

  • You can send a written notification about treatment to the hospital TPA or at the nearest insurer’s branch. For emergency hospitalizations inform the insurer immediately
  • After getting discharged, you can collect your medical investigation reports, discharge summary, and all the treatment-related documents from the hospital
  • Submit all the required documents in original at the TPA’s office within 7 days of hospital discharge
  • Collect and submit the final hospital bills, claim form, discharge summary, doctor's prescriptions, invoices, investigation reports, and pharmacy bills
  • Pre and post-hospitalization expenses can be also be claimed later. Do not forget to provide all the documents within 7-days of post-hospitalization treatment

  • All the bills and receipts
  • All the diagnostic reports
  • Pharmacy bills
  • Doctor’s prescriptions

  • Discharge report
