The scheme is implemented through the Atal Amrit Abhiyan Society, with Medi Assist Insurance TPA Pvt. Ltd. as the Implementation Support Agency (ISA) for processing claims and providing IT support.

      Why Was Atal Amrit Abhiyan Needed?

      Assam is a state where a large portion of the population lives in remote tea garden communities, riverine islands, and border districts, hours away from any specialist hospital. When cancer or a cardiac condition strikes, families do not just face a health crisis. They face a financial one. Selling land, borrowing from moneylenders, or travelling to Kolkata or Delhi for treatment has been the reality for decades.

      Atal Amrit Abhiyan was designed specifically to address this. The state committed ₹200 crore to implement the scheme, and beneficiaries can access treatment at empanelled hospitals across five major Indian cities, not just within Assam.

      Eligibility

      Atal Amrit Abhiyan is open to two clear categories of beneficiaries:

      BPL Families:

      • Must be a permanent resident of Assam
      • Annual family income must be below ₹1.2 lakh
      • Must hold a valid ration card issued under the National Food Security Act (NFSA) by the Assam government
      • Coverage is completely free, zero premium, zero enrollment fee

      APL Families:

      • Must be a permanent resident of Assam
      • Annual family income between ₹1.2 lakh and ₹5 lakh
      • Must have valid documentation establishing APL status
      • Pay a nominal premium of ₹100 per member per year

      Who is NOT eligible:

      • Current or retired employees of the Central Government or State Government and their families
      • Families with annual income above ₹5 lakh
      • Residents of other states, Assam residency is mandatory

      Every eligible adult member gets an individual Atal Amrit Card. Minors are enrolled under the card of the head of family.

      Registration Process

      Enrollment under Atal Amrit Abhiyan can be done offline through district-level enrollment centres.

      Step 1: Download the Atal Amrit Abhiyan Application Form from the official NHM Assam portal at nhm.assam.gov.in, or collect it directly from the District Enrollment Centre

      Step 2: Fill out the form with accurate personal and family details

      Step 3: Attach all required documents, identity proof, income certificate, ration card, and a doctor-attested photograph of the patient

      Step 4: Submit the completed form at the Office of the Joint Director of Health Services of your district. Forms can also be submitted by post

      Step 5: Once verified and approved, the Atal Amrit Card, a laminated smart card with a unique identification number, is issued to each eligible adult member of the family

      Enrollment is also facilitated at regular health camps organised across districts. APL families must pay ₹100 per member at the time of enrollment, which is renewed annually.

      Benefits and Coverage

      This is what makes Atal Amrit Abhiyan genuinely impactful. Coverage is not for routine illnesses, it is specifically focused on the six high-cost disease groups that have historically pushed families into financial ruin.

      Six Covered Disease Groups:

      • Cardiovascular diseases- cardiac surgeries, heart procedures, bypass, valve replacement
      • Cancer- chemotherapy, radiation therapy, surgical oncology
      • Kidney diseases- dialysis, kidney transplant, related procedures
      • Neonatal diseases- conditions affecting newborns requiring specialist neonatal care
      • Neurological conditions- brain and neurological surgeries, stroke care
      • Burns- treatment for burn injuries

      Coverage highlights:

      • ₹2 lakh per individual per year- cashless at empanelled hospitals.
      • 438 approved procedures across the six disease specialties.
      • Pre-existing disease coverage- conditions that existed before enrollment are covered.
      • Transport allowance of ₹3,000 per year- ₹300 per visit, for travel to the empanelled hospital.
      • Daily allowance of ₹1,000 (up to ₹10,000 maximum) for treatment taken outside Assam.
      • Cashless at the point of care- no advance payment required; the card settles the bill.

      Pregnant women covered under Atal Amrit Abhiyan can also claim benefits under Janani Suraksha Yojana (JSY), Pradhan Mantri Matru Vandana Yojana (PMMVY), and Surakshit Matritva Aashwasan Yojana (SUMAN) for delivery incentives, maternity cash support, and free specialist antenatal care respectively. TB patients can simultaneously claim ₹1,000 per month through Nikshay Poshan Yojana. Where treatment costs exceed ₹2 lakh, Rashtriya Arogya Nidhi (RAN) provides additional one-time assistance of up to ₹15 lakh at government super-specialty hospitals.

      Documents Required

      Keep the following ready before visiting the enrollment centre:

      • Voter ID card or any valid government-issued identity proof.
      • EPIC card, for adults; birth certificate in original for minors.
      • Income certificate, to establish BPL or APL status.
      • NFSA ration card issued by the Assam government, valid as eligibility proof.
      • Photograph of the patient, attested by a doctor.
      • Mobile number for updates and notifications.

      Card Download Process

      Once enrollment is approved, the Atal Amrit Card is issued physically at the enrollment centre or district office. For digital access:

      • Visit the official portal at atalamritabhiyan.aaas-assam.in
      • Log in with your unique identification number issued at enrollment
      • Navigate to the card download section
      • Enter your registered details and download the card

      The card can also be checked and verified at the same portal. Each card carries the beneficiary's unique identification number, photograph, and family details and is accepted at all empanelled hospitals across Assam and partner cities.

      Claim Process

      Atal Amrit Abhiyan works on a fully cashless model. The card settles everything at the point of care, no advance payment, no reimbursement paperwork.

      • Visit any empanelled hospital with your Atal Amrit Card and identity proof.
      • The hospital verifies eligibility through the Medi Assist ISA system.
      • Pre-authorisation is obtained before planned treatments begin.
      • All bills are settled directly between Medi Assist and the hospital.
      • For treatment outside Assam, a daily allowance of ₹1,000 (up to ₹10,000) and transport allowance of ₹300 per visit (up to ₹3,000 per year) are provided.

      Families with members having Autism, Cerebral Palsy, or Multiple Disabilities can additionally explore the Niramaya Health Insurance Scheme. Children in institutional care under Mission Vatsalya and adolescent girls under SAG who are Assam residents can also access Atal Amrit benefits at empanelled hospitals. Tamil Nadu residents have CMCHIS and AP residents have Dr. YSR Aarogyasri as parallel options.
