      Why Was Swasthya Sathi Needed?

      There are no warning bells before any medical emergencies. One day, a family can be living comfortably, and the next, they might need someone to go to the hospital for a period of time or an operation, and they are left in financial ruin because of that. In West Bengal, families who live on a low income or in rural areas have historically been forced to sell their assets, take out loans or simply not seek out the care they need when they develop a serious illness.

      At Swasthya Sathi, the vision is to ensure that no family in West Bengal has to sacrifice quality health care services while making a living. As per the West Bengal Budget 2024-25, the scheme is currently active for 2.45 crore families in the state, in which 2,749 hospitals are empanelled, and so far 65 lakh beneficiaries have availed services worth ₹8,600 crore since its inception. Every day, around 6,000 patients use Swasthya Sathi facilities. Those are not just numbers, it's about the family that was cared for and were given the support that they needed the most at that moment.

      Who Is Eligible for Swasthya Sathi?

      From December 2020, the scheme offers near-universal coverage to all residents of West Bengal. The eligibility criteria are broad and inclusive:

      • Must be a permanent resident of West Bengal.
      • All residents, regardless of age, caste, religion, or profession, are eligible.
      • ASHA workers, ICDS workers and helpers are specifically included.
      • Members of Self-Help Groups (SHGs) qualify.
      • Contractual employees in certain categories also qualify.
      • Existing beneficiaries of Rashtriya Swasthya Bima Yojana (RSBY) are covered.

      Who is NOT eligible:

      Individuals already enrolled under any other government-sponsored health insurance scheme are not eligible to duplicate coverage under Swasthya Sathi.

      There is no age restriction. A newborn and an 80-year-old grandparent are both covered. And the scheme explicitly includes in-laws and dependent physically challenged family members, a provision that most health insurance schemes, private or public, often overlook. Families with members having Autism, Cerebral Palsy or Multiple Disabilities may also explore the Niramaya Health Insurance Scheme, which provides specialised coverage beyond what Swasthya Sathi covers for this group.

      Benefits and Coverage

      This is one aspect which makes Swasthya Sathi different from several schemes for health. The coverage is truly comprehensive:

      • Secondary and tertiary medical expenses, such as surgeries, hospitalisation, complex procedures, and health cover of ₹5 lakh per family per year.
      • Disease coverage from the start no waiting period, no exclusions.
      • TB patients whose treatment is covered under Swasthya Sathi hospitalisation can also claim nutritional support through Nikshay Poshan Yojana.
      • There is no cap on the family size; irrespective of how large the family size is, every member of the family will get ₹ 5 lakh.
      • This feature of cashless hospitalisation includes the cost of doctor's consultation, diagnostic tests and medicines that are incurred within 1 day prior to admission and 5 days after discharge.
      • Zero premium cost, the state government will pay the premium; the beneficiary will not pay a premium.
      • Maternity-related hospitalisation covered under Swasthya Sathi can also be combined with benefits available under Janani Suraksha Yojana (JSY) for eligible women. Pregnant women availing delivery care under Swasthya Sathi may also be entitled to direct cash assistance under Pradhan Mantri Matru Vandana Yojana (PMMVY).
      • Transport allowance is ₹200 at discharge, and up to ₹700 for treatment at government hospitals.
      • Over 2,000 medical packages covered, with rates fixed by the Package Selection Committee for transparency.
      • Free food during hospitalisation for beneficiaries treated at government hospitals.
      • SMS alerts are given as real-time notifications about hospitalisation, treatment status, and discharge.
      • Pre-authorisation within 24 hours, which means all treatment procedures are pre-approved digitally within 24 hours.

      What is NOT covered:

      • OPD consultation charges are not covered; however, if an OPD visit leads to admission, it is covered.
      • Treatments at non-empanelled private hospitals
      • Cosmetic procedures

      Registration Process

      Registering for Swasthya Sathi is straightforward. The government also runs a Duare Sarkar (Government at Your Doorstep) campaign periodically, where you can enrol at your local camp without visiting any office.

      Online Registration:

      • Visit the official portal at swasthyasathi.gov.in
      • Click on "Apply Now" on the homepage.
      • Fill in the application form with accurate personal and family details.
      • Upload the required documents.
      • Submit the form, followed by a verification process, and the smart card is issued after approval.

      Offline / Camp Registration:

      • Visit a Duare Sarkar camp in your area when announced.
      • Carry your documents. You will be assisted by an officer with the application on the spot.
      • The smart card is typically issued on the day of enrolment at camp.

      Every beneficiary family receives the Swasthya Sathi Smart Card on the day of enrolment. The card contains the family's biometric details, photographs, mobile number, and SECC ID.

      Documents Required

      The documents needed are basic:

      • Aadhaar card- primary identity proof; mandatory
      • If Aadhaar is not available, any government-issued photo ID card
      • Domicile certificate- to establish West Bengal residency
      • Ration card- if from a low-income family
      • Recent photographs of all family members
      • SECC ID card- if applicable
      • Mobile number for SMS alerts and updates

      Card Download Process

      Once your application is approved and your Swasthya Sathi Smart Card is ready, you can download a digital version:

      • Visit swasthyasathi.gov.in
      • Click on "Beneficiary Search" or "Download Card" on the portal.
      • Enter your Aadhaar number or registered mobile number.
      • Verify via OTP
      • Download and save your e-card.

      The smart card has a lifetime validity and is renewed automatically every year. No annual renewal visit is required.

      Claim Process

      Swasthya Sathi works on a cashless model, meaning the beneficiary does not pay the hospital and claim later. Here is how it works:

      • Visit any empanelled hospital with your Swasthya Sathi Smart Card.
      • The hospital verifies your identity using the biometric details on the card; the fingerprint of any enrolled family member can be used.
      • The hospital applies for pre-authorisation online, which is approved within 24 hours.
      • Treatment begins. All costs are settled directly between the hospital and the Swasthya Sathi system.
      • Post-discharge, your e-health record is updated on the portal in real time.
      • The hospital receives claim reimbursement within 30 days of the scheme.

      If, for any reason, you need reimbursement after paying out of pocket at an empanelled hospital, you can approach the district health authority with bills and documents for processing.

      Fraud protection: The system uses AI and human audit checks to flag unusual patterns like excessive admissions, blocked packages, or unnecessary procedures, protecting both beneficiaries and the public fund.

      Swasthya Sathi has a structured support mechanism for any problem that arises while at the hospital, while registering or with the card:

      • Official Portal- swasthyasathi.gov.in
      • Online Grievance Portal- The complaints can be filed directly through the official online grievance portal.
      • Swasthya Sathi Mobile App- Available on Google Play Store. It assists beneficiaries in verifying empanelled hospitals, the status of their card, and to lodge complaints.
      • A district-level helpdesk is available at all District health offices in West Bengal.

      All online grievances are recorded and have a specific timeframe to be resolved. Any hospital that denies service and/or requests payment is in violation and must be reported.

      Why Swasthya Sathi Matters

      This is a very big thing because it is a health insurance plan that covers all residents irrespective of their income, caste or family size, does not require any premiums and has no paperwork to be done at the hospital and has a cover of ₹5 lakh per year. Swasthya Sathi is the difference between care and non-care for a child requiring heart surgery in Murshidabad and an elderly woman needing a knee replacement in Purulia, and getting a daily wage job to earn a living. Also adolescent girls covered under the Scheme for Adolescent Girls (SAG) through Anganwadi Centres can transition to Swasthya Sathi coverage as they grow older and start families.

      The scheme is not foolproof, and there are coverage gaps; not all hospitals are empanelled, and awareness is also not uniform in remote locations. However, as a pledge from the State Government to support the health of its citizens, it is one of the more comprehensive state-level health insurance programmes in India.
