If the original registration certificate (RC) of your motor vehicle is damaged, lost, completely written-off, or torn, you can apply for a duplicate copy of the same by using Form 26. It is a duplicate vehicle registration certificate form that helps you get a duplicate RC for your vehicle so that you can continue driving it legally without any worry.
For vehicles that are under a lease or hire-purchase agreement, RTO Form 26 must be submitted in two copies, where one copy can be kept by the financier. And, if the original registering authority is different, the form must be submitted in three copies.

Vehicle owners whose original RC has been lost.
Vehicle owners whose RC has been torn, or has become unreadable.
Vehicle owners whose RC has been stolen.
Vehicle owners whose RC has been completely destroyed due to accidents, fire, or other incidents.

Step 1: Visit the Parivahan Sewa website.
Step 2: Go to to the 'Informational Services' tab and click on the 'Downloadable Forms' option.
Step 3: You will see a list of multiple RTO forms. Find and click on the 'Form 26' link to download it.

Step 1: Mention the name of the concerned RTO.
Step 2: Enter the vehicle registration number for which the duplicate RC is being requested.
Step 3: Specify the reason for applying for a duplicate RC, such as lost, destroyed, torn, soiled, or written-off.
Step 4: Fill in the declaration confirming that the vehicle registration has not been suspended or cancelled.
Step 5: Attach the damaged RC, if available.
Step 6: Indicate whether the vehicle is under hire-purchase, lease, or hypothecation. If it is financed, you will need details and consent from the financier.
Step 7: Enter your mobile number and add your signature wherever required.
Step 8: After completing the form, submit it to the concerned RTO along with the required documents and applicable fee.
You are required to attach the following documents with the RTO Form 26:
Copy of the FIR if the original RC is lost or stolen
Copy of the valid insurance policy of the vehicle.
Valid PUC or Pollution Under Control certificate
Aadhaar card, voter ID, or passport for identity and address proof
Challan clearance certificate
Damaged RC, if applicable
NOC from the financier if the vehicle is on lease/hypothecation.

Form 26 can be rejected for the following reasons:
The form is incomplete, or some details are missing.
Wrong vehicle number or owner details are written in the form.
The reason for the loss or damage to the RC is not clearly stated.
Required documents are not attached with the application.
A copy of the FIR is not attached in case the RC is lost or stolen.
Financier's NOC is not attached if the vehicle is under loan or hypothecation.
The vehicle registration is suspended, cancelled, or under dispute.
Pending road tax, challans, or other dues are unpaid.
The form is not signed by the registered vehicle owner.
The use of RTO Form 26 is to obtain a duplicate copy of your vehicle registration certificate in case the original RC is lost, stolen, written-off, or torn.
