      What is Form 45?

      Form 45 is a request application used to get a permit for a commercial tourist vehicle under the provisions of the Central Motor Vehicles Rules (CMVR), 1989. The vehicle owner or operator must submit this form to the Regional Transport Office (RTO). It contains important information about the vehicle, the owner, and the type of permit required. The authorities use the information given in this form to assess whether the vehicle qualifies for a tourist permit. After all details are verified, the RTO issues the permit, allowing the vehicle to function as a commercial tourist vehicle.

      What Details are Included in Form 45?

      Here are some of the key detailsthat are included in Form 45:

      • Applicant details, such as the name, classification (individual, corporation, or cooperative society, etc.)
      • Information about the father/husband of the individual applicant and information about the partners (if the applicant is a firm/company)
      • The applicant's complete address, along with supporting documents such as the ration card, electricity bill, etc.
      • A copy of the partnership deed or memorandum of association must be included if the applicant is a company or firm.
      • If the applicant wants to drive the vehicle, then the applicant's driving licence number, validity, name and address of the licencing authority should be provided.
      • Vehicle registration certificate (RC), along with information about its first registration date and vehicle insurance certificate number.
      • Details regarding the vehicle, such as its make and model, and the tourist permits associated with it, etc.
      • Particulars of suspensions, convictions, or cancellations, if any.
      • Self-declaration concerning the submission of the vehicle registration document and the details of the fee paid.
      • Signature or thumb impression of the applicant.

      How to Download Form 45?

      • Step 1: Open the Parivahan Sewa website.
      • Step 2: Go to the 'Informational Services' tab. Click on 'Downloadable Forms'.
      • Step 3: Look for Form 45 in the RTO forms list shown on the screen.

      How to Fill Form 45?

      To easily fill out Form 45, adhere to these steps:

      • Step 1: Specify the location of the State Transport Authority.
      • Step 2: If the application is for obtaining a permit for specific states, provide their names.
      • Step 3: Enter applicant-related information, including name and classification (individual, corporation, partnership, etc.).
      • Step 4: Provide additional information about the applicant, such as the applicant's husband's name if the applicant is an individual, or details about the managing partner/director if the applicant is a partnership firm.
      • Step 5: Mention the full address and include suitable proof, such as attested copies of the ration card, electricity bill, etc., for individual applicants. Add certified copies of the memorandum of association or partnership deed if the applicant is a company/firm.
      • Step 6: If the applicant intends to drive the tourist vehicle, the driving licence details, including the driving licence number, date, and validity period, must be provided.
      • Step 7: Include information about the vehicle registration certificate, such as the date of its first registration and registration certificate number.

      • Step 9: List the vehicle's details, including type, make, and model.
