What is Form 68?

Form 68 can be used to appeal the results of a vehicle fitness assessment conducted at an Automated Testing Station. It allows vehicle owners to request a reassessment of the test outcome if they believe it is incorrect or unfair.

The applicant needs to add information about the vehicle, test station details, and other relevant information. The RTO will examine the appeal and may uphold the original result or order a re-test/reassessment, in accordance with the Central Motor Vehicles Rules, 1989.

How to Obtain Form 68?

Form 68 is submitted by the vehicle owner to the appropriate transport authority. Applicants can download the form from Parivahan Sewa. After submission, the appeal is reviewed by the RTO officials.

What Details are Included in Form 68?

A few essential details that must be included in this form are listed below:

• Personal details of the applicant, like name, contact number, and email ID.
• If the applicant is the registered owner of the vehicle (Yes/No).
• Vehicle details, including registration number, chassis number, make, and model.
• Test station details such as station name, station address, and date of test.
• If the vehicle was tested at another station (Yes/No). If yes, details about the same.
• Details of the plea against the test results, including information about vehicle alteration/repair/adjustment, since the last test (Yes/No). If yes, more details about the modification need to be provided along with a copy of the test result.

• Step 1: Visit the Parivahan Sewa portal.
• Step 2: Click on 'Downloadable Forms' under the Informational Services tab.
• Step 3: From the list of RTO forms displayed on the screen, tap on 'Form 68'.
• Step 4: Download the form.

How to Fill Form 68?

You can use these guidelines to fill out the details on Form 68:

• Provide the applicant's personal details, including name, contact number, and email ID.
• State Yes/No, to indicate if the applicant is the registered owner of the vehicle.
• Include vehicle details, such as the vehicle registration number, chassis number, make, and model.
• Provide the testing station details, including the station name and address.
• Mention the date of the test.
• Indicate whether the vehicle was tested at another station by answering Yes or No. If it was, add the relevant details.
• Include any information regarding your plea against the test results, especially if there have been any alterations, repairs, or adjustments made to the vehicle since the last test.
• State Yes or No to confirm if modifications were made. If modifications have been made to the vehicle, the details of those modifications must be added.
• A copy of the test result must be attached to the form.
