• Number of samples/vehicles seized
• Details of the batch or production volume related to the samples
• Details of the witnesses interviewed for the investigation (if any), such as their name, designation, department, and signature
• Details of any other document enclosed with the form
• Names and signatures of both the manufacturer/importer/their representatives and the investigating officer
