This document certifies that the employee has been working with the organisation since a specific date and that the company has offices in more than one state or UT. It is submitted with Form 27A, which is the application form for a BH number plate.

      • Step 1: Go to the official Parivahan Sewa portal.
      • Step 2: Click on the 'Informational Services' menu at the top.
      • Step 3: Click on the 'Downloadable Forms' option.
      • Step 4: Find and click on the 'Form 60' link to download it in the PDF format.
