What is Form 64?

Form 64 is an official application form used by Automated Testing Stations (ATS) to apply for, or renew, an ATS registration certificate. Automated vehicle-related services using special equipment and systems can only be conducted by the ATS after it has received the registration certificate.

The form collects details about the applicant, infrastructure, and testing facilities. The concerned regional transport office (RTO) reviews the information provided to determine whether the applicant meets the eligibility criteria set out in the Central Motor Vehicles Rules, 1989, before granting or renewing the registration certificate.

How to Obtain Form 64?

Form 64 is an application form that must be submitted to the concerned RTO for seeking approval to establish or operate an Automated Testing Station. Applicants can obtain the form through the Parivahan Sewa portal. The application must be completely filled in and signed by the applicant, then submitted to the appropriate RTO for processing. After submission, the relevant transport authority assesses the application to determine whether to approve or extend the registration certificate.

Form 64 generally includes the following information:

• Name, address, telephone, and email ID of the applicant
• CIN/Shop Act Registration Number/Udhyam Aadhaar Number
• PAN and GST number
• Status of the applicant (Company/Firm/Trust/Society/Proprietor/Government/JV/PPP/Others)
• Number of lanes
• Land details (Location and details of ownership)
• Number of employees
• Vahan API application number and date
• Duly signed and dated undertaking by the authorised signatory (owner and/or operator)

How to Download Form 64?

You can download Form 64 by following the steps given below:

• Step 1: Visit the Parivahan Sewa portal.
• Step 2: Click on 'Downloadable Forms' under the Informational Services tab.
• Step 3: From the list of forms displayed on the screen, tap on 'Form 64'.
• Step 4: Download the form.

How to Fill Form 64?

Follow these steps to fill out Form 64:

• Fill in the name, address, telephone, and email ID of the applicant
• Fill in the CIN/Shop Act Registration Number/Udhyam Aadhaar Number
• Enter the PAN and GST number
• Tick the appropriate box to indicate the status of the applicant (Company/Firm/Trust/Society/Proprietor/Government/JV/PPP/Others)
• Choose the right box to indicate the number of lanes
• Provide details about the land on which the ATS is located (Location and details of ownership).
• Provide information about the number of employees.
• Enter the application number and date.
• The owner and/or operator (authorised signatory) must sign the form.

• Duplicate of the initial registration certificate
• Duplicate of the successful pre-commissioning audit and evaluation report

Common Reasons for Rejection of Form 64

• Inaccurate or Partial Information: If the form has inaccurate information or lacks details, the application might be denied. Candidates must examine all sections thoroughly before passing the form to the RTO.
• Absence of Required Documentation: If you do not submit the required documents, such as evidence of land ownership and a copy of the current registration certificate, the authorities may reject the form.
• Failure to Adhere to Technical Specifications: If the Automated Testing Station fails to adhere to the specified technical standards, the application might be denied.
• Previous Registration Invalid or Expired: In case of renewals, inaccurate or outdated registration information could lead to denial.
• Missing Signature: Applications that are not signed or properly completed may be considered invalid by the authority.
