• Step 3: Click on 'Downloadable Forms' from the list of options available.

How to Fill Out Form 5A?

Driving schools and authorised personnel must follow these steps to complete Form 5A accurately:

• Step 1: Enter the applicant's details, including full name, father's/husband's name, Aadhaar number, and residential address.
• Step 2: Mention the enrolment date and record the applicant's admission serial number as per the register.
