      RTO Form 5 is a certificate provided by an accredited motor driving school. It serves as legal proof that you have successfully completed a driving course from a certified driving school as per the rules laid down by the transport authority. It also indicates that you have obtained the skills needed to handle specific classes of transport (commercial) vehicles.

      Who Needs to Submit Form 5?

      Form 5 is not required for all drivers, such as private car (LMV-NT) drivers or two-wheeler riders. However, Form 5 is mandatory for:

      Transport Vehicle Applicants: Drivers intending to drive taxis, auto-rickshaws, goods carriers, or other transport vehicles.

      Heavy Motor Vehicles (HMV): Drivers willing to drive trucks, trailers, and large buses.

      Hazardous Goods Carriers: It is required for those transporting fuel, chemicals, or explosives.

      Why is Form 5 Important for Your Driving Licence?

      According to the Central Motor Vehicles Rules, 1989, applicants seeking a driving licence for transport or commercial vehicles must undergo training from a recognised driving school. Form 5 serves as proof that the applicant has successfully completed this training.

      It ensures that the driver of a heavy truck, bus, or taxi has received proper training before applying for a transport vehicle licence. The program generally covers the following:

      Vehicle mechanism and maintenance

      Advanced traffic rules and signalling

      First aid and emergency response

      Safe driving practices in different road and weather conditions

      Without Form 5, the licencing authority may not process an application for a transport vehicle driving licence.

      What Details are Included in Form 5?

      The following are the details included in Form 5:

      Issuer's Information: Includes the name, address, and licence number of the motor driving school. For the RTO to accept the form, the school must be registered and authorised by the state transport department.

      Trainee's Details: It includes the full name, father's name, address, signature, and the enrolment date of the applicant/trainee, along with the serial number as registered in Form 14 of the driving school.

      Training Period: The dates for the start and end of the training course are indicated on Form 5.

      Vehicle Class: The type of vehicle on which the candidate has been trained is indicated on the form, e.g., Heavy Goods Vehicle, Passenger Transport Vehicle.

      Instructor's Declaration: The qualified instructor signs and declares that the candidate is proficient in driving and has knowledge of the required rules.

      Step 1: Visit the official website of Parivahan Sewa at parivahan.gov.in.

      Step 2: Click on 'Informational Services' on the navigation bar.

      Step 3: Tap on the 'Downloadable Forms' option from the drop-down menu.

      Step 4: From the list of forms, click on the option given for 'Form 5' to download it.

      How to Obtain Form 5 From a Driving School?

      You can also obtain Form 5 from a driving school by following the steps given below:

      Enroll in a Government-Recognised School: Ensure the driving school is authorised to provide training for transport vehicles. You can check their credentials on the Parivahan Sewa website or at your local RTO.

      Complete the Training: You are required to complete the required training hours for both theoretical and practical training modules. It usually comprises a theory test and a practical exam.

      Clear the Internal Assessment: Before getting Form 5, you have to undergo a test conducted by the school to ensure you know how to operate the specified class of vehicle.

      Collect the Certificate: You can collect your Form 5 after successfully completing the internal assessment test.

      You can download Form 5 from the 'Informational Services' section of the official Parivahan Sewa website. Go to the 'Downloadable Forms' page, and tap on the option for 'Form 5' to download it on your device.

      Form 5 is required only for driving licences issued for commercial vehicles. Individuals applying for a driving licence for their personal vehicles do not need to submit this form.

      You can only download the blank Form 5 from the Parivahan Sewa portal but it must be filled, signed, and stamped by a certified motor driving school to be valid.

      Form 5 is used as proof that a person has completed professional training from a recognised driving school before applying for a commercial vehicle driving licence.

      If you lose your Form 5, you must approach the driving school that issued it. They can issue a duplicate copy based on their training records.
