The Addressee: Address this form to your Regional Transport Office (RTO) where the vehicle is currently registered.
Current Owner's Details: Name, signature, and address of the vehicle owner on whose name the vehicle is registered.
Vehicle Details: Engine number, make, registration number, and chassis number or motor number for battery-operated vehicles.
Authorised Dealer's Information: Name, place of business, authorisation certificate number, issuing authority, and validity of authorisation.
Declaration: A declaration stating that there are no pending taxes or challans on the vehicle.
Date of Delivery: Date of delivery of vehicle to the authorised dealer.
