      What is Form 54?

      Form 54 is an Accident Information Report (AIR) required under Rule 150 (1) and 150 (2) of the Central Motor Vehicles Rules, 1989. A registering authority or a police officer must provide accident-related information in Form 54 to the person eligible for compensation. It must be provided within 7 days of receiving the application, after the applicant has paid the application fee of ₹10.

      What Details are Included in Form 54?

      These are the details that an RTO Form 54 contains:

      • Legal Aspects: Name of the police station where the case is registered, FIR No./CR No./traffic accident report, and sections applied.
      • Accident Details: Date, time, and place of accident.
      • Details of the Victim: Name and full address of the injured/deceased, and name of the hospital.
      • Vehicle Details: Registration number of your vehicle, type of vehicle, class of vehicle, engine number, or motor number (for vehicles that run on battery), and chassis number.
      • Driving Licence Particulars: Number, expiry date, issuing authority's address, route permit or Licence of use details, and badge number in case of public service vehicles.
      • Vehicle Owner: Name and address of the vehicle owner.
      • Insurer Details: Name and address of the insurance provider and its divisional office.
      • Insurance Policy Particulars: The number of insurance policy or insurance certificate, and their date of validity.
      • Action Taken: Details about the actions taken on-site and their results.
